• Bring your insurance card(s) with you to your appointment. (You may be required to pay a deposit prior to seeing the physician if you cannot present your insurance card at the time of your visit.)
• Secure a referral from your primary care doctor (if your insurance requires)
• Be prepared to pay your co-payment for the day’s services
